Assessment

Assessments are personalized for each patient and the fees below are estimates. Final pricing for assessments may change depending on the needs of the patient. You may not know what type of assessment is best for your child, and that’s ok. We will figure that out together during the intake visit. We are committed to transparency in pricing. The cost estimate below includes all appointments related to the evaluation.

Note that an assessment does not guarantee a diagnosis.

All assessments include:

  • Virtual intake appointment (parent/caregiver only)

  • In-person testing visit

  • Review of records

  • Personalized recommendations and report

  • Virtual feedback appointment

Assessment

Rates

Ages 4 & younger. Includes developmental testing, evaluation of attention and possible ADHD when appropriate, parent report measures, and autism measures. Other concerns related to mood (e.g., anxiety and depression) will also be evaluated.

Ages 5-16. Includes IQ testing, evaluation of attention and possible ADHD, parent report measures, and Autism measures. Other concerns related to mood (e.g., anxiety and depression) will also be evaluated.

Autism Evaluation

Ages 17-20. Includes IQ testing, evaluation of attention and possible ADHD, parent/caregiver and self report measures, personality measures, and Autism measures. Other concerns related to mood (e.g., anxiety and depression) will also be evaluated. 

Autism Evaluation

Ages 5-16. This assessment focuses on whether a child has ADHD or cognitive concerns and includes IQ testing and evaluation of attention and executive functioning (including parent report measures) but not Autism testing. 

ADHD Evaluation

Ages 7-16. Learning disability (including dyslexia) or giftedness testing; includes IQ and achievement testing and evaluation of attention and executive functioning, including parent report measures. ADHD testing is included. This does not include Autism testing.

Ages 7-16. Evaluation for autism, mood/behavior disorder, and/or attention and learning difficulties; includes IQ and achievement testing, autism testing, evaluation of attention and executive functioning (i.e., ADHD testing) and parent report measures.

PayMenT

Unlocked Potential does not accept insurance. However, we are happy to provide you with a Superbill (i.e., a detailed receipt) that you can provide to your insurance company for potential reimbursement or submit for FSA/HSA reimbursement.

We are proud to partner with the WV Hope Scholarship and can be found on the EMA portal.

We accept payment by credit/debit card or check. At the time of booking, we require a card to be placed on file. You have the option of changing to a different mode of payment at the time of the visit. Full payment is due on the date of service.
